Our San Bernardino Street Medicine Teams increase access to healthcare for underserved communities, especially for people who are housing insecure. We meet people where they are, on the street, in encampments, in shelters, in rural areas, and who are otherwise excluded from traditional healthcare systems. We provide ongoing care for our patients.

San Bernardino is the largest county in the United States. It encompasses large areas that are both suburban and rural. In rural areas there is a distinct lack of health, wellness, and shelter resources for vulnerable populations. This creates a challenge to serve the people who have the highest needs and when encampment sweeps happen, we work diligently to find our patients, especially as many don’t have reliable means of communication. We do this by talking with our patients about potential secondary locations where we might reach them and by working with our partners and communities who also serve the same population.

Street Medicine Services

Direct primary care medical services tailored to the immediate needs of the unsheltered, which reduces visits to the ER, including wound care, chronic disease management, and infectious disease monitoring.

Prevention, Testing, and Treatment: Rapid point-of-care HIV testing, pre-exposure prophylaxis (PrEP), and immediate initiation of antiretroviral therapy (ART) for people living with HIV. Ongoing follow-up supports adherence and viral suppression, with telehealth access to HIV specialists when indicated.

Intensive, person-centered care coordination for high-need patients, including comprehensive assessments, individualized care plans, coordination across providers, and connection to community supports that address the social drivers of health.

Guidance and support in navigating healthcare systems to secure necessary coverage.

Screening, assessment, diagnosis, psychiatric consultation, medication management, and Medication-Assisted Treatment (MAT), including buprenorphine and naltrexone. All services are trauma-informed and integrated into the primary care encounter.
We also provide overdose prevention including Naloxone.

Confidential support for individuals' mental well-being, including referrals for counseling and therapy.

Point of Care testing in the field including HIV, Syphilis, Hepatitis C, Strep, and HbA1c.

Convenient prescribing access and delivery of necessary medications through mobile services.

We provide essential health and hygiene supplies to promote healthy habits, a sense of wellbeing, and to help achieve wellness.
